This recent article presented at Digestive Diseases Week reinforces what our practice already knows: for irritable bowel syndrome and functional dyspepsia “a multidisciplinary approach “was superior for global symptoms, improvement in specific functional symptoms, psychological state, quality of life, and cost” to care from a gastroenterologist alone.”

“When specialists collaborated, 84% of patients met the primary end point of global symptom improvement, compared with 57% of those managed by standard care, according to the researchers.

 

“There is evidence to support psychological, behavioral and dietary therapies in functional disease, but these are still offered to only a minority of patients,” said Dr. Basnayake, whose group submitted their findings for presentation at the 2020 Digestive Disease Week (abstract 409), which was canceled because of the COVID-19 pandemic.”

 

…“Functional disorders are one of the most common reasons to consult with a gastroenterologist. Typically, a series of diagnostic procedures are conducted to rule out malignancy or another organic disease, but symptoms are poorly controlled with drug therapies that are typically prescribed,” Dr. Basnayake said. Due to the low level of long-term response, he said the advantages of multidisciplinary care demonstrated in this trial provide the basis for more widespread application.

 

These data are “not surprising if you do this every day,” said Brian E. Lacy, MD, PhD, a professor of medicine at Mayo Clinic in Jacksonville, Fla. Dr. Lacy, who has published frequently about functional GI diseases…

“The teaching point is that functional disorders arise from a number of different reasons that include gut inflammation, changes in the gut microbiome, stress, and disorders of the brain–gut axis. Thus, changing diet, working on stress, incorporating biofeedback, and even hypnosis might be helpful in improving symptoms,” he said.
